Free Care Plan Request

After submitting your details, our team will review your case and provide a personalized Biohealth Care Plan with guidance on your current dental situation, a cost estimate and a recommendation for next steps for your Dental Biohealth improvement journey such as the before and after example images below.

For our Biohealth experts to complete your Biohealth Care Plan, you will need to provide a 2D panoramic X-Ray photo less than 12 months old, either via the upload form option in this form or emailed to reception@swiss-biohealth.swiss . These can be obtained from your local dentist. Images must be in PDF, JPG, JPEG, PNG or TIFF format.

Note: the preliminary remote plan you get does not represent a medical diagnosis. For further assessment we recommend a clinic visit on site.
